Case Management Services program provides coordinated case management services for patients with a chronic condition, catastrophic injury or illness requiring complex services and treatment. Our Case Managers work closely with benefit managers, program administrators, families, and providers to coordinate delivery of high quality, medically necessary services.  Optima MMG assists with coordination of care and applying individualized benefits (when necessary) that result in lowered healthcare costs while providing necessary services, respecting the dignity of the patient and ensuring quality outcomes. We offer care management programs for all ages besides a physician-run program with significant nursing support.
